Naval Health Clinic Hawaii

You can request a copy of medical records if:

  • You are going to a foreign (OUTSIDE THE US)
  • You are a Marine and PCSing to an operational unit, or a Sailor going to a ship.

If you are in this category you must have:

  • 1) A copy of orders that say unit is operational. 
  • If the medical records office has a hard copy, they will give it to you. If they do not, they will give you an electronic copy. If you are PCSing to a non-operational unit, you will fill out DD Form 877 at your new clinic. Clinic will fax this form to KBAY, KBAY will send to new clinic. 

EAS or Retirement:

  • MARINES: Request hard copy > Take copy to IPAC > DD214
  • NAVY: Does not hand carry, records are retired. KBAY can send an e-copy upon request.

  • Moving without spouse
    • Power of Attorney (PoA) for Accept/ Terminate Base Housing, Car Shipping, DMO, Real Estate Purchase, etc.
  • Terminating Quarters (On/ Off Base)
    • Written notice + copy of orders
  • PCS to foreign country
    • Report Birth to U.S. Embassy to ensure citizenship

 

Sponsorship Request & Assistance

  • Submit to Bldg. 579 or your S1
  • Liaison at future duty assignment
  • Assist/ arrange temp lodging, transportation from airport, provide info on checking in, reporting procedures, documents to have, etc.

Disabled American Veterans (DAV)

This is recommended for those getting out of service. The DAV rep will review your medical record and discuss how to file for VA medical benefits post active duty. This is by appointment only in building 244.

Medical Records:

Dependents can request a copy of immunizations and last physical. Dependents must request records upon arriving at their New Military Treatment Facility or medical provider. Visit Family Medical Records 30 days prior to detaching from command for copies of medications and immunizations.

  • Transfer your Prime Enrollment with just a phone call. Whether stateside or overseas, you can transfer enrollment with just one phone call to your new location. You don't need orders; there are no waiting lists or forms to complete. 
  • When you get to your new duty station, just pick up the phone and call your regional contractor or log on to tricare.mil.
